| The Department was at the above referenced site to conduct a Routine Inspection. The Operator has built the site and is in the process of moving the drill rig on site to begin drilling.  The Operator did not have the ESCGP-1 on site during this inspection. The Operator utilized an existing access road to bring the drill rig on site.  The access road is in terrible condition.  There is no way emergency vehicles could make it up the road.  There are no E/S controls on either side of the excavation along the access road.  The Department is requesting a meeting on site to discuss the ESCGP-1.  This site has the potential to pollute the waters of the Commonwealth.  The Department was told the existing road was going to be used, not widen it by 25 feet.  Violations will be issued if the site does not match the ESCGP-1.  Please contact the Department to schedule a site meeting.

| The Department was at the above referenced site to conduct a Routine Inspection.  The Operator has built the site and is in the process of moving the drill rig on site to begin drilling.  The Operator did not have the ESCGP-1 on site during this inspection.  The Operator utilized an existing access road to bring the drill rig on site.  The access road is in terrible condition.  There is no way emergency vehicles could make it up the road.  There are no E/S controls on either side of the excavation along the access road.  The Department is requesting a meeting on site to discuss the ESCGP-1.  This site has the potential to pollute the waters of the Commonwealth.  The Department was told the existing road was going to be used, not widen it by 25 feet. Violations will be issued if the site does not match the ESCGP-1.  Please contact the Department to schedule a site meeting.

2036499 Primary Facility 2012-01-04 Routine/Complete Inspection No Violations Noted The Department was at the above referenced site to conduct a Routine Inspection. The Operator has built the site and is in the process of moving the drill rig on site to begin drilling. The Operator did not have the ESCGP-1 on site during this inspection. The Operator utilized an existing access road to bring the drill rig on site. The access road is in terrible condition. There is no way emergency vehicles could make it up the road. There are no E/S controls on either side of the excavation along the access road. The Department is requesting a meeting on site to discuss the ESCGP-1. This site has the potential to pollute the waters of the Commonwealth. The Department was told the existing road was going to be used, not widen it by 25 feet. Violations will be issued if the site does not match the ESCGP-1. Please contact the Department to schedule a site meeting.
