The Engineer was on site to make sure the contractors were following the construction standards of 3:1 slopes and also checking the 8 inch lifts. The contractors were installing drains on site so the springs did no impact the well pad. On 08/09/16 at approximately 1430 hours I conducted a routine complete inspection of the Chevron Appalachia LLC Kieffer well site. The Act 9 sign was in place and all required documents were present at the site. The well pad was fenced, gated, and locked at the time of inspection. Chevron had recently submitted a minor modification and Environmental Engineering Specialist Mohamed Sam found that the last major modification for the site was approved on 12/21/2010. As a result, the ESCGP-1 expired on 12/20/2015 with no record of an extension being requested or approved. An NOT was completed by WQS Ben Dillie on 10/09/2014 and in the report Dillie indicated the NOT was denied due to the well pad having not been reclaimed to post construction design. A violation of 102.7(c) has occurred for failure to terminate or renew the ESCGP.
