| On Monday, June 12, 2017, I conducted a routine-complete inspection at the NV20 Well Site located in Morris Township, Greene County.

The Act 9 sign was in place and clearly legible at the site entrance.  The access road and the well pad were in good condition.  The site is stabilized with greater than 70% perennial vegetative growth.

The mailbox used to store required documentation on-site contained some debris but no documents.  The ESCGP associated with the NV20 Well Site was terminated in January 2016 and the gas wells have been completed and in production for some time.  Although the well permits, ESCGP and ESCGP Plan are not required to be kept on-site at this time, the operator should consider keeping an as-built drawing with associated PCSM controls as related to the site restoration plan on-site.  The Emergency Response and PPC Plans, however, are required to be kept on-site for the duration of operations.  The operator should replace the missing Emergency Response and PPC Plans upon receiving this

2610636 Primary Facility 2017-07-05 Routine/Complete Inspection No Violations Noted On Monday, June 12, 2017, I conducted a routine-complete inspection at the NV20 Well Site located in Morris Township, Greene County.

The Act 9 sign was in place and clearly legible at the site entrance. The access road and the well pad were in good condition. The site is stabilized with greater than 70% perennial vegetative growth.

The mailbox used to store required documentation on-site contained some debris but no documents. The ESCGP associated with the NV20 Well Site was terminated in January 2016 and the gas wells have been completed and in production for some time. Although the well permits, ESCGP and ESCGP Plan are not required to be kept on-site at this time, the operator should consider keeping an as-built drawing with associated PCSM controls as related to the site restoration plan on-site. The Emergency Response and PPC Plans, however, are required to be kept on-site for the duration of operations. The operator should replace the missing Emergency Response and PPC Plans upon receiving this

2812433 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted On Thursday December 6, 2018 I conducted a routine-complete inspection of the NV20 Well Site located in Morris Township, Greene County.

The gas well and the associated infrastructure appeared to be in good condition and functioning properly. The gas well was tagged with identification.

No violations were identified during the inspection.
