2325611 | Primary Facility | 2014-11-25 | Incident- Response to Accident or Event | No Violations Noted | On 11/22/2014 Mark Barbier (Anadarko) reported approximately 20 barrels of produced fluid and associated production gas was discharged (uncontrolled to the atmosphere) from the 4H well due to a pipe joint failure on the wellhead. According to the call, the well was shut in manually by Anadarko personnel. |
2575434 | Primary Facility | 2017-03-21 | Routine/Complete Inspection | No Violations Noted | Routine inspection of unvonventional well pad in production. A-6H well is plugged with no detectable methane emissions. Bubbling noted on location, later discovered to be a phenomenon of the thawing ground. Methane emissions not detected by GES during sampling of location. |
2578539 | Primary Facility | 2017-03-23 | Routine/Complete Inspection | No Violations Noted | Routine inspection of plugged well. 0% methane detected at time of inspection. No violations noted at time of inspection. |
2581726 | Primary Facility | 2017-03-28 | Follow-up Inspection | No Violations Noted | Follow up inspection of an unconventional well pad in production. Some bubbling still noted on the pad. 0% combustible gas emissions detected at time of inspection. No violations noted at time of inspection. |
2584273 | Primary Facility | 2017-04-12 | Follow-up Inspection | No Violations Noted | Follow up inspection of location and wells following discovery of a bubbling phenomenon on location. Phenomenon appears to have subsided, No methane detected during inspection. No violations noted at time of inspection. |

2643965 | Primary Facility | 2017-10-04 | Routine/Complete Inspection | No Violations Noted | Weather was 57° and sunny at time of inspection. Well is plugged with a candycane style vent. Well vent had 0% combustible gas detected at time of inspection. |
2687535 | Primary Facility | 2018-01-22 | Incident- Response to Accident or Event | No Violations Noted | Spill response inspection; 1/22/2018.
Mark Barbier (ARD Operating) called the Department at 18:01 on 1/20/2018 to report the release of 176 gallons of a 30% methanol/water solution. The methanol/water mixture was released from a failed ball valve on the 4H(081-20563). 250 gallons of the released methanol/water mixture and melt water was vacuumed from the 3H well cellar later that night. At approximately 13:45 on 1/22/2018, I met onsite with Mark Barbier and Rick Hall (both ARD). They showed me the failed valve leading to the 4H well and the excavated flow path into the 3H cellar. Material from the flow path across several feet of the pad surface had been excavated and placed in a overpack drum for disposal. The material under the excavation was frozen solid. I was only able to drive a digging bar 1/4 inch into the frozen surface. Water was streaming into the well cellars from the well pad surface due to warming temperatures and runoff. I was told the cellar would continue to be vacuumed. The conditio |
