A review of the Off-Site Waste Disposal information documented in the report was conducted and no deficiencies were identified. Due to snow cover, compliance with 25 PA Code § 102.22 (a)(2)(i) Permanent stabilization could not be verified at this time. 25 PA Code § 102.22 (a)(2)(i) states for an earth disturbance activity or any stage or phase of an activity to be considered permanently stabilized, the disturbed areas shall be covered with a minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation. A follow-up inspection will be conducted to determine if this site meets the stabilization requirements when field conditions allow.
2386039 Primary Facility 2015-07-06 Site Restoration No Violations Noted The Department inspected the SWEPI LP – Cummings 823 site on 7/6/15 @1300 hours in response to receipt of a Notice of Termination received by the Department on 7/1/2015. The weather was 75° and mostly cloudy. No temporary E&S controls were observed. All slopes appeared to contain at least 70% vegetative cover. This inspection meets the requirements of 25 PA Code §102.22(a)(1)(2). No violations were cited during this inspection. This NOT is recommended for approval at this time.
2654747 Primary Facility 2017-10-31 Routine/Partial Inspection No Violations Noted I got electrical conductivity readings from all the secondary containments and 2 puddles on the pad with an Oakton PCSTestr 35; all were <0.35 mS/cm.
2775712 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP - Cummings Unit 823 Well pad located in Chatham Twp, Tioga Co. on 9/17/2018 @1200 hours. The weather was 72 degrees, cloudy, winds N 11mph. Present for the inspection was myself and Korey Starkey, DEP Env Trainee.

The access road, well pad and E&S controls were observed. An adequate site ID sign was observed at the entrance to access road. The well pad appears to be in production, however, all pressure gauges attached to production equipment registered 0 PSI, so the wells do not appear to be producing at this time. One rolloff tank was observed on secondary containment located on the southern corner of the pad. All puddles on the pad surface were screened for field conductivity and found to be <1000 uS/cm.

The area surrounding the pad appeared stabilized by >70% vegetative cover. No erosion or sedimentation was observed. No violations are noted at this time.

We departed the site at 1245 hours.

2810069 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Cummings 823 Well Pad in Chatham Twp, Tioga County on 12/4/2018 at 1200 hours. The weather was partly cloudy and 28 degrees.

There are currently 6 wells onsite with 6 production tanks. All fluids on pad surface were within normal conductivity ranges. There were some higher conductivity readings but were in secondary containment. Pad appears to be stabilized and vegetated.

No violations noted at this time. I departed the site at 1220.
