2318187 | Primary Facility | 2014-10-28 | Routine/Complete Inspection | No Violations Noted | 12:10, calm, sunny, temperatures in low 70's. Site was busy with the Orion 15 rig on site other contractors included: Halliburton (mud engineering) and Cathedral (directional Drilling). Conducted inspection of the site with company man, Joe. There were no issues on the pad at the time of my inspection nor were there any issues with E&S controls on site. |

2363170 | Primary Facility | 2015-04-21 | Routine/Partial Inspection | No Violations Noted | 4 Marcellus Shale wells Fractured in January 2015. Flowed back in March 2015. Highest conductance reading today 1.5 mS/cm from pooled water on pad surface. |
2376313 | Primary Facility | 2015-06-03 | Routine/Complete Inspection | Pending | Puddles on the well pad surface and water within the sediment basins adjacent to the pad were screened for electrical conductance using a Oakton multi-parameter PCSTester 35. Elevated conductance was observed in all puddles on the pad (2.8-6.8 mS/cm) and the sediment basin (2.92 mS/cm) adjacent to the northeast corner of the well pad. No stressed or dead vegetation was observed surrounding the pad, between the pad and sediment basin, or downslope of the sediment basin. |
2434025 | Primary Facility | 2015-12-18 | Administrative/File Review | Violation(s) Noted | Violations cited agains 113-20265 for likely unreported spill. |
2436283 | Primary Facility | 2015-12-28 | Drilling/Alteration | No Violations Noted
2437763 | Primary Facility | 2016-01-05 | Routine/Complete Inspection | No Violations Noted | Flowback operations are underway for all four wells. All equipment, lines, and tanks are within containment. The electrical conductance of fluid puddles within the containment surrounding the well heads was over the instrument reading (20 mS/cm) limit of my Oakton Multi-parameter PCSTestr™ 35. The amount of fluid present within the containment should be monitored and removed as necessary. No areas of staining on the pad surface were observed.
No violations were cited during this inspection. |
2440575 | Primary Facility | 2016-01-14 | Routine/Complete Inspection | No Violations Noted
2490920 | Primary Facility | 2016-06-01 | Routine/Complete Inspection | No Violations Noted | Vegetative cover on site is less than 70%. Temporary erosion and sediment control BMPs are in place and in good repair. No evidence of accelerated erosion or sediment control was observed.
Approximately 30 brine water tanks are located within containment on site. A number of the tanks are full , and tanker trucks are regularly arriving to empty the tanks. No evidence of spills such as staining, elevetated electrical conductivity, or salt crust/crystals was observed. This site is currently not compliant with the Department's regulations pertaining to site restoration (Title 58 Pa. C.S. § 3216). The Department's records indicate that a request for a 2-year extension of time for site restoration was filed on 02/16/2016; this request has not yet been approved. No violations were cited during this inspection. |
2542052 | Primary Facility | 2016-12-06 | Routine/Complete Inspection | No Violations Noted | Arrived @ 09:30. Weather: cold, cloudy, ice and patchy snow on the ground. The 4 wells on location were in production at the time of my inspection. Annuli were plumbed into two 550 gallon poly tanks. No combustible gas was detected at the vent of either polytank (Altair 5X Gas Meter calibrated 12/6/16). Left @ 10:10. |

2716689 | Primary Facility | 2018-02-20 | Routine/Complete Inspection | No Violations Noted | Fours wells staged for production phase. Four each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. Methanol tank located near well heads. No violations noted. |
2817826 | Primary Facility | 2018-12-18 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection with Jared Jacobini, DEP Water Quality Specialist at 14:24; the weather was ~30°F.
There were 4 properly tagged Marcellus wells staged for production. There were 4 GPUs, 4 produced water tanks and 1 fresh water injection tank onsite and 1 compressor in secondary containment. The GPU and produced water tank secondary containment was about half full with ice. The produced water tanks had some moderate pitting/corrosion (Photo #4). The labels on the water injection tank were faded and illegible. On the east side of the pad there was a large area of erosion in the collection ditch leading to an infiltration basin (Photos #6, 7). It is recommended that the area be stabilized/restored as per Department guidelines. There is also a swale entering the opposite side of the infiltration basin that had some ice. This area may need attention to ensure that it is functioning as intended. The infiltration basin itself was nearly filled with water and ice. The basin may need to hav |
