1967796 | Primary Facility | 2011-04-22 | Routine/Complete Inspection | Violation(s) Noted | Today Jon Scott and I inspected this gas well site in follow-up to Jon's 4/13/2011 inspection to determine if the operator had corrected the E&S concerns that Jon had told them about in a voice mail on 4/13/11. Upon inspection we found the operator had made some improvements, however the E&S controls remain in a state of disrepair. The slit fence is down in (2) locations on the north edge of the site and significant slit has left the slit traveling over 50 yards down slope. Also the slit fence is down on the east edge of the site with slit leaving the site as well. I called Jordan Florida, the local Rep for O&R Drilling and setup an on-site to review the problem for 4/25/2011. |
1967979 | Primary Facility | 2011-04-25 | Follow-up Inspection | Outstanding Violations - No Viols Req'd | Today Scott Lux and I completed a follow-up inspection of this gas well site to determine if the operator repaired the E&S controls and slumping pit liner as required during our 4/23/2011 phone conversation, and to review what actions need to be taken to prevent further acclerated erosion. Upon inspection we found the operator had repaired the slit fence on the north edge of the site and also pulled up and secured the pit liner as required. However the slit fence remained down on the east edge of the site and the accerlated erosion is still occurring. After walking the site perimeter with the operator, the operator decided to eliminate vechile traffic on the well site access road and redirect the sheet flow from the fields on the west side of the site, to the access road. This work would be done after the operator installs (3) water bars to catch and hold the water on the road. Also the operaor will repair the down slit fence , and dredge/cleanout (3) sediment basins. |
1972088 | Primary Facility | 2011-05-09 | Follow-up Inspection | No Violations Noted | Today Scott Lux and I conducted a follow-up inspection of this site to determine if the violations identified on 4/22/11 have been corrected. Upon inspection we found the violations corrected. |
1976681 | Primary Facility | 2011-05-16 | Follow-up Inspection | Violation(s) Noted | Today's inspection was in follow-up to my 5/9/2011 inspection to determine if the E&S controls that have been installed have been maintained as required. Upon inspection I found the pits had been clsoed and the site remains unstable. The extensive nature of the earth moving for both the access road and the site , combine with the slope creates accerlated erosion that is being managed by water bars on the access road, sump pits on the site and slit fence. Upon inspedctiontoday i found the access road erosion being managed. However the site erosion was not. Sediment from the site has filed the sump and is overflowing with the water outflow, overburdening the slit fence. The slit fence is down once again on the north end of the ite and sediment has left the site. I will call the operator and issue an NOV. |
1978012 | Primary Facility | 2011-06-06 | Follow-up Inspection | No Violations Noted | Today's inspection was in follow-up to my 5/16 inspection and 6/1 phone conversation will company owner Ohannes Karaoghlanian to determine if the BMPS have been repaired as required. Upon inspeciton I found all slit fence reinstalled and all (4) sediment basins cleaned out. I also recieved the revised E&S plan as requested. This site needs reclaimed or steps taken to stabilze the soil must be taken; the pit area is not completely reclaimed. |
1985000 | Primary Facility | 2011-06-28 | Follow-up Inspection | Violation(s) Noted | Inspection revealed the drill pit reclaim was not completed according to the requiermetns in the regulation. An NOV was sent. |
1985028 | Primary Facility | 2011-06-27 | Follow-up Inspection | Violation(s) Noted | Follow-up inspection to determine if the site BMP are being maintained and check the status of the site and pit reclaim. Upon inspection I found the BMP are not being matined as required and the site reclaim work has not changed sincew my last inspection on June 6 2011. This even though i had cautioned the operator that he must keep the BMPs up and finish the reclaim to reduce potential problems. |
1985194 | Primary Facility | 2011-06-29 | Follow-up Inspection | Outstanding Violations - No Viols Req'd | On-site with Curtis LeSuer to collect samples of liquid flowing from hole in drill pit liner and from on top of the liner. |
1989224 | Primary Facility | 2011-07-15 | Routine/Complete Inspection | Outstanding Violations - No Viols Req'd | Scott Lux and I met on-site with Operator/owner Ohannes Karolighain, and consultant to review the violations identifed in previous NOV's. Also discussed the need for action plan submittal by 7/29/2011. |
1995391 | Primary Facility | 2011-08-01 | Follow-up Inspection | Outstanding Violations - No Viols Req'd | Met dirt contractor on-site to review action plan to clean-up old pit and stabilze ground surface. Dirt contractor will be digging a new waste pit and transfering the old liner and pit contents. I instructed him to call me for a inspection of the old pit area and the new pit before flling in and closing. |
1995498 | Primary Facility | 2011-08-03 | Routine/Complete Inspection | No Violations Noted | Today's inspection was to verify the work completed to clean-up and relocate the waste pit contents at this well site. I met the dirt contractor on-site and inspected the old pit area and the new pit , before covering and closure. Upon inspection I found the work completed as required. The area still needs covered and stabilzed with seed and mulch. |
2001466 | Primary Facility | 2011-08-25 | Follow-up Inspection | No Violations Noted | Follow-up inspection to check on status of reclaim- the well pad has been graded and part of the lengthy access road pushed back- some grass growing on access road. top soil still needs to be pushed back on well pad and some of the road, everything needs and seeded/mulched. |
2004619 | Primary Facility | 2011-09-22 | Follow-up Inspection | No Violations Noted | Follow-up to determine if the access road and well pad have been reclaimed and stabilzed as required. Upon inspection I found the access road back to orginal grade with mulch and grass seed applied. The well pad site was also graded, mulched and seeded. However a top soil pile on the south edge of the site and (2) sumps on the north edge of the site remain on-site. I called the dirt contractor Jordan Flordia and he stated he left these in place intentionally because he expected the well gas pad site will be reworked when fracing for the utica. |
2014414 | Primary Facility | 2011-10-27 | Site Restoration | No Violations Noted | Site restoration inspection- the access road was back to grade and have 80 % grass cover- no e&s problems- the well pad was reclaimed and grass was at 80% no E&S problems- small top soil pile still remains on south edge of well pad- one sump remains on the north edge of well pad- |
