Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1796671 2009-05-06 Pending Site ID sign in place. E&S plan on site. Well being drilled at time of inspection.

Silt fence installed along down-slope side of site adequately maintained. Site is stabilized with rock. No signs of accelerated erosion on site. Access road ok. Pit appears sound and had adequate freeboard..

  • There are concerns that this site requires an ESCGP-1 permit due to greater than five acres of disturbance included in this well site, access
roads, other connected wells included in this project, etc.. Follow-up inspections will be conducted to determine the amount of total
disturbance, and to determine if the sites are in violation. Culvert installed on access road requires rip-rap at the outlet to minimize erosion. Please address this concern and contact this office upon completion.

1798023 2009-05-14 Violation(s) Noted Site ID sign in place. E&S plan on site. Well not equipped for production (well head only on site).

Silt fence installed along down-slope side of site adequately maintained. Pit appears sound and had adequate freeboard Site is stabilized with rock. No signs of accelerated erosion on site. Access road ok. Inspection revealed the following deficiencies:

1) Failure to obtain ESCGP-1 permit. When the total amount of earth disturbance activity results in 5 acres or greater of substantially
connected earth disturbance at one time or 5 or more acres of earth will be disturbed over the life of the project the operator must obtain
an ESCGP-1 approval before conducting any earth disturbance activities. The total disturbance for your project was approx. 15.08 acres. In addittion to the ESCGP-1 permit that needs to be submitted and approved for the Auen well project as stated above, the culvert installed on the access road to this site requires rip-rap at the outlet to minimize erosion. Please address this concern and contact me.
