2597850 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted Routine inspection of the Gerhart Marcellus well pad. Site is occupied by a single completed well, the Gerhart #1H (37-033-27111). This well has only had the tophole section drilled, with no lateral completed, is capped off with a blind flange on casing head, and has never been placed in an active production status. The Ceritas Midstream Partners-Karthaus West gathering pipeline originally planned to serve the site was cleared and grubbed, but was never built, and the pipeline ROW has been fully restored and pipeline ESCGP terminated.
The original ESCGP for this site was modified to upgrade the existing sediment traps into infiltration basins, and the post-construction pad footprint was approved at the original full buildout size. The location is built entirely on reclaimed bituminous mine spoils, and while ponded water may be present on the pad itself after a rain event, runoff off the aggregate pad quickly infiltrates into the surrounding vegetated spoil material. The ESCGP for this site also incl
2724143 Primary Facility 2018-04-27 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at BKV Operating, LLC Gerhart C-19 Marcellus well pad on April 27, 2018 with environmental consultant Kevin Brady. There is one Marcellus well (033-27111) drilled on this pad. The well is currently in regulatory inactive status and this status will expire on 2/23/20. The well head is tagged. Bubbling outside the casing was observed in the well cellar. This case will be referred to a Department Oil & Gas Inspector for a follow-up inspection regarding the bubbling. According to Mr. Brady this well was drilled but never fracked. The well casing is currently filled with an oil base drill mud.

The well pad remains the original footprint size 430' x 330'. All PCSMs are in place. The ESCGP (ESX11-033-0024) for the construction of the well pad and access road was terminated by Department staff on 10/25/16. In addition, a well site restoration report for this well was approved on 10/18/16 by Department staff.
