Operator representatives Jason Canose and Matt Streams notified Department at approximately 1150 via phone of water leaking out from between the conductor and casing of a conventional well. Water appears to be tinted orange with a flow rate calculated to be approximately 2 gal/hr.

Operator is actively investigating incident and is going to construct a containment unit around wellhead to hold any discharged fluids.

Referred to OGI Supervisor Ed Ritzer for follow-up.

Operator appears to be in compliance with current policy at this time.

2240937 2014-01-29 No Violations Noted
2241490 2014-01-22 Pending Opeartaor reports a fluid discharge from well: No discharge at time of inspection. Gas present at conductor surface annulus. operator to sample fluids and determine source of gas.
2242593 2014-02-03 In Compliance with Policy Admin inspt as follow-up to inspt record 2238846 in which fluids reported to be leaking out from between the conductor and casing of a conventional well.

Department notified via phone by Operator representative Jason Canose that an unknown quantity of additional fluids have "bubbled out" into previously constructed containment unit (record 2241490). Approximately 40 gallons of accumulated ice/snow/discharged fluids transferred from containment unit into production tank for appropriate disposal.

Fluid samples collected on 1/29/2014 by Operator. Results shall be submitted to Department upon Operator's receipt as previously requested.

Operator appears to be in compliance with current policy at this time.

2248177 2014-02-20 No Violations Noted
2248641 2014-02-20 Pending RTPT inspt as follow-up to inspt record 2238846 in which fluids reported to be leaking out from between the conductor and casing of a conventional well.

Inspection conducted with OGI Rick Hoover. Operator rep Jason Canose also on-site.

At time of inspection, fluids being discharged between 11 ¾” X 8 5/8” annulus into previously constructed containment unit (record 2241490). Per Operator, containment unit routinely emptied with accumulation transferred to production tank for appropriate disposal.

Fluid samples from 11 ¾” X 8 5/8” annulus collected on 1/29/2014 by Operator. Results submitted to Department on 2/13/2014. Due to lack of parameters tested, additional sampling was scheduled for today.

Split samples collected by Department and Environmental Service Laboratories. Samples collected from 11 ¾” X 8 5/8” annulus and from 8 5/8” X 4 ½” annulus.

Gas odor noted. Associated inspection record 2248177

Inspection is currently pending awaiting sample analysis and Operator investigation as to source of discharge.

2248645 2014-02-20 In Compliance with Policy Conference call held between OGI Rick Hoover, WQS Samantha Foulk, and XTO reps Jason Canose and Mike West as requested by Opr in response to inspt record 2238846 in which fluids reported to be leaking out from between the conductor and casing of a conventional well.Opr and Dept discussed possible scenarios as to why gas and fluids are being discharged from the 11 ¾” X 8 5/8” annulus.Operator intends to monitor annular gas for two weeks. Monitor will identify gas and record levels of each type (i.e. CO2, methane, etc.). Mr. West stated records of surrounding wells were inspected and no abnormalities were observed. Mr. West also stated that well tender checks well every day and no fluids were reported to be discharged over the weekend. Department informed Operator that fluids were being discharged today (inspt record 2248641).Operator agreed to collect isotopic samples of gas. Mutual decision between Operator and Department to wait for results of fluid samples (inspt record 2248641) to be received before developing and implementing an action plan.
