2427605 Primary Facility 2015-08-12 Follow-up Inspection Pending Met with John Rushin on location, found well on a slightly-benched area on hillside, near old access road. Well has 7" casing swaged to 2 1/2" tubing for total height of ~5 feet. Significant leak at collar-tubing connection above swage where tubing is broken above threads (weight of drip appears to have aided in tubing break). Gas fills the air around wellhead; 62% gas/100% LEL readings @ break. Gas flow is not under pressure, as no hiss was heard . Long 7" drip is parted, but not leaking, indicating valves @ wellhead are closed. Pipeline from well is on surface and is damaged in several places with old, failed plastic pipe patches. John took new GPS reading. There is no ID on well.
2484941 Primary Facility 2016-05-17 Follow-up Inspection Pending Follow up inspection of gas well near Roberts Road; Eighty Four, PA 15330

There appears to be no change from last inspection. Well is leaking from a break in a section of pipe approximately 3” in diameter connected to a vertical well head approximately 6” in diameter and about 4 ½ ft. high. There is a strong hydrocarbon odor several yards from the break in the pipe and the sound of leaking gas. The well is located in a wooded area. There is a significant amount of pipe (approximately 2” in diameter) on the ground connected to the well. There is no tag on the well.
