2423914 Primary Facility 2015-10-20 Routine/Complete Inspection No Violations Noted On 10/20/15, the Department conducted an inspection of the SWN PU GG Fries Pad. The Department had approved of the P&A of the Fries 4H (115-20811) which consisted of filling the conductor pipe with concrete. Conductor only drilling to have occurred on this pad. Today’s inspection was to evaluate SWN’s reclamation of the pad. The pad still exists, along with associated infiltration berm. Top soil has been spread on the pads surface and has vegetation established at about a 60% coverage. I explained to Mr. Gulyas that for the pad site to be considered eligible for termination, that the pad would need to be eliminated and that the site would need to be re-established to near its original contours.
2511154 Primary Facility 2016-08-17 Routine/Complete Inspection No Violations Noted On 8/17/16, the Department met on-site at the SWN Fries Pad with SWN representatives to discuss the various options SWN has in terminating the ESCGP for the Fries Pad. Present during the inspection were myself, Rob Marquez, Department Project Manager, Jason Petlock, SWN Manager, Rich Gulyas, SWN Construction Manager, Ryan Klemish, SWN Regulatory Analyst, and Andy Breese, SWN Construction Manager. SWN has indicated that the land owner would like to keep the pad. To that affect, SWN has submitted a minor modification application to the Department, which if approved, would allow the pad to stay in place and allow for the termination of the permit. Today’s inspection found that the top soil spread on the pads surface, and subsequent seeding and mulching was effective, and that there is greater than 70% vegetative cover on the pad surface. The Fries 4H was successfully P&A last year.
