At the time of my inspection Ziegenfuss Drilling was on site. Spoke with Lenny Long (Ziegenfuss) who was spudding conductor on the 4H and preparing to move to the 3H. Had replaced 20” conductor on the 1H with 24”, and would follow with the 2H. There were no violations at the time of my inspection. Left location @ 10:40.
2563563 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted Routine Inspection; 2/15/2017.

I entered the site off Sugar Camp Road at approximately 10:00. At the time of the inspection, conductor was set on four of the five wells onsite. The Ziegenfuss rig was packed-up but still onsite. A vac-truck was onsite cleaning out the single Frac tank. One diesel tank was onsite in containment and two roll-off containers onsite were holding cuttings from the conductor. The two center wells were labeled the Winter 1 (081-21474) and Winter 2 (081-21501), while the two additional wells were not labeled. Please label the remaining two wells as soon as possible (I understand the conductor were just set).

The well pad is well vegetated except for the recent construction of PCSM (Post Construction Stormwater Management) features on the eastern (lower) part of the site. On 2/1/2017, the Department approved a permit modification to add additional gravel pad surface to the well site. This construction of this additional impervious area has not been initiated. No Violations

2605247 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted Routine inspection; 6/15/2017.

I entered the site at 14:15 and talked to the company man (Carey Carr). They were in the process of tripping out of the Winter 1 (081-21474) to replace the BHA mud motor. Planning on finishing this third casing string before reaming run with the same diameter. Containment is in good condition and housekeeping looks good. Please be mindful of the chemical storage area. Chemicals are stored on containment as required but i recommend not storing chemicals so close to the edge when possible (see photos). The site is seeded and stabilized and no E&S issues were noted.

2623888 Primary Facility 2017-07-26 Drilling/Alteration No Violations Noted Arrived @ 15:19, weather: cloudy. Met with company man Carrey Carr. Orion 15 drilled to 659’ on air with 22” hammer. Casing shoe to be set at 645’. At the time of my inspection, crews were preparing for a reaming run with surface casing cement job to follow. Left @ 16:00.
2623937 Primary Facility 2017-07-27 Drilling/Alteration No Violations Noted Arrived @ 11:00, weather: cool, cloudy. Met with company man Carrey Carr. Encountered water from 430’ – 470’ while reaming last night (approx. 150 bbls). circulating with water when I arrived. This was a 4-string design with 18-5/8” surface casing. At the time of my inspection, Orion 15 was circulating prior to start of cement job and Haliburton was onsite. Pumps were on at 12:26. Plan was to pump 800 sacks, and get 60 bbls back on gauged hole. Dropped plug @ 13:18. Returns to surface stopped and started seveal times from 13:20 to approximately 13:30, when 80 bbls were displaced with 80 bbls to go. At 13:33 there was a substantial color change from reddish brown to cement brown-grey. At 13:36, returns were still gritty with cuttings. 13:39 good cement returns, cement job finished at 13:53. There were no violations observed during this inspection. Left at 14:00.
2625558 Primary Facility 2017-08-14 Drilling/Alteration No Violations Noted Arrived @ 12:30, weather: humid, cloudy. Met with company man Justin Foster. Orion 15 at 8772’, building the curve. Expect to be done in 4 days or so. Second Intermediate casing cement job went well. Inflection does a step-wise FIT test to 18 ppg equivalent mud weight. Left @ 13:10.
2635732 Primary Facility 2017-09-06 Routine/Complete Inspection No Violations Noted I entered the site with Clark Smith (DEP) and we talked to Greg Malekos (Inflection) before the inspection. At the time of the inspection, Inflection was drilling the Winter 2. Rig mats were very uneven on the north and east sides of the site apparently due to compaction and traffic on this recently expanded part if the pad. The areas under the rig and associated equipment did not show signs of settling. A sheen and some friction reducer lubricant was visible on water in containment near the compressors. I recommend vacuuming the water from containment. I also recommend vacuuming and careful management if water in containment when rigging-down.

I recommend evaluating the areas where the pad has settled after the rig moves offsite and before completions operations begin. No violations noted.

2640503 Primary Facility 2017-09-27 Drilling/Alteration No Violations Noted Weather: cloudy, humid. Arrived at 10:15. Orion 15 is off site, moved to the Hensler pad. Mats were being pressure washed in prep for cleaning and leaving the site. OVerall the mats were coated with a thick layer of mud and water. The purpose of my inspection was to check for any bubbling on the 18-5/8" x 13-3/8" annulus of the Winter 2H. The cellar of the winter 2H was filled to within 3' of its top with a mix of water and mud. The well cellars for the 1H and 4H looked similar. No bubbling was observed in any of the cellars. There were no violations noted during this inspection.
2649069 Primary Facility 2017-10-18 Drilling/Alteration No Violations Noted Arrived at 2:22. Weather: sunny and warm. There are three wells on this pad. Frac heads were installed on all three. Coiled tubing rig [Key energy services] on site. Met with Jerry Roberts, company man and Jeff Whitehair, consultant. At the time of my inspection, coil tubing was preparing for a clean up run, on rhe 2H and planning to tag and verify top of cement. Plan is to run DFIT on Winter 2H and 4H then do a whole frac stage on the Winter 1. There were no violations observed st the time of this inspection.
2650968 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted I met onsite with Jerry Roberts (company man) and then walked the site. At the time of the inspection, a Coiled Tubing rig was onsite rigging-up on the Winter 2 (081-21501). One tear in the containment liner was noted west of the well heads where rig mats were removed. The pad is uneven from the rig traffic and the condition of the containment remains unknown. Since the containment used for drilling will be reused for completions, I recommend individual containments around tank batteries (working tanks, etc.) and the blender. A modular fresh water tank is being installed on the auxiliary pad east of the well heads. Fresh water will be pumped to the site via temporary overland waterlines from the Bennett and Stunner-Smith sites. No violations noted.
2659030 Primary Facility 2017-11-09 Drilling/Alteration No Violations Noted Arrived at 12:30. Weather: rain. Met with Greg Malekos, drilling superintendent. Fraccing Winter 1. Doing approximately 6 stages a day. Plan to move to Winter 2H tomorrow, and last to Winter 4. Discussed need to keep mud off Sugar Camp. There were no violations observed st the time of this inspection.
2666736 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted I met with Jeff Whitehair on location. Cudd coil tubing rig currently drilling out plugs on the 1H well. Containment appeared to be clean and in excellent condition. A puddle (5-10 gallons) present near the company man trailer showed slightly elevated conductance (2.3 mS/cm); I recommended to Jeff that he have it vacced. I also noted a small (less than a gallon) puddle of a white liquid on containment behind the coil tubing truck, and I also recommended to Jeff that he have it cleaned. Once they are finished with the 1H, Inflection plans to move to the 4H and then the 2H to finish drilling plugs.
2671718 Primary Facility 2017-12-14 Routine/Complete Inspection No Violations Noted Arrived at 14:00. Weather: cold windy and sunny. Three well heads wrapped in plastic: Winter 1H, 2H, 4H. Allison crane crew on site getting ready for flow back. No violations observed at the time of this inspection.
2676357 Primary Facility 2017-12-19 Routine/Complete Inspection No Violations Noted At the time of the inspection, the wells were drilled, completed, and awaiting flow-back. The secondary/auxiliary pad at the entrance was being prepared for an Aboveground Storage Tank (AST). The production facilities and flow lines were also being installed. Perimeter E & S controls were installed downslope of earthmoving activities. Overall the site was very busy. No violations noted.
2682080 Primary Facility 2018-01-11 Drilling/Alteration No Violations Noted Arrived at 12:20. Weather: warm. Flow back ops began on 1/10/18. All 3 wells made gas early. Currently going through flow back subcontractor's separators and to pipeline. Met with Jeff Whitehair (frac consultant). No violations observed at the time of this inspection.
2686073 Primary Facility 2018-01-23 Drilling/Alteration No Violations Noted Arrived at 10:19 Weather: rain, temps in upper 40s, ice on pad. Met with Brian Twigg (Inflection). Removing separators and moving to full production. Three wells being put into production on this location. All are 4-string casing design. Winter 2 burping soap from second intermediate annulus - 13-3/8" x 9-5/8" (see photo), as reported yesterday by Greg Malekos. Cellar was sucked down so that annular relationships could be seen. Small amount of gas was emerging from the 18-5/8" x 13-3/8" annulus (previously perfed and squeezed).

The burping was noticed yesterday. Previous cold temps may have Frozen annulus so that it was frozen shut. The cement top in this second intermediate annulus was left down. No bubbling or burping was observed at the the WInter 1H and Winter 4H wells.

2687849 Primary Facility 2018-01-29 Drilling/Alteration No Violations Noted Arrived at 9:15. Weather sunny, cold. Purpose: to check on 13-5/9" x 9-5/8" annulus pressure build for Winter 2H. Met with Brian Twigg (Inflection), who said 72-hour PBT led to 214 psi. Will collect iso samples later this week after annular vents installed.
2689808 Primary Facility 2018-02-01 Routine/Complete Inspection No Violations Noted Routine Inspection; 2/1/2018.

At the time of the inspection, three wells were in production and temporary sand separators were still connected. Sand separators were plumbed to frac tanks in containment on the southeast corner of the well pad. Containment was being installed around the production units and produced water tanks were in containment. Earth disturbance activities are ongoing to restore the site (except the AST). Unable to determine the origin of the 2 annulus vents at time of inspection. Outer most vent had 0% combustible gas detected. Innermost event had 100% methane and 100% LEL of combustible gas detected at time of inspection. The well had an analog pressure gauge attached to the tubing above the swab valve and a digital pressure gauge attached to the 5 1/2 inch casing. The analog pressure gauge was a 0 to 10 K gauge that read approximately 350 psi. The digital gauge on the casing read 621 psi. The well number of 4H was stamped on the wellhead but a well tag with the API number could not be located at time of inspection. No violations noted at time of inspection.

There were no Inflection representatives on location at time of inspection. The well had 4 annulus vents plumbed to surface that were labeled 13, 9, 18 and 24. The vent labeled 13 had 85% methane and 100% LEL of combustible gas detected at time of inspection. The vent labeled 9 had 45% methane and 100% LEL of combustible gas. The 2 vents labeled 18 and 24 had 0% combustible gas detected at time of inspection. The well had an analog pressure gauge attached to the tubing above the swab valve and a digital pressure gauge attached to the 5 1/2 inch casing. The analog pressure gauge was a 0 to 10 K gauge that read approximately 50 psi. The digital gauge on the casing read 476 psi. No violations noted at time of inspection.
