Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1874526 2010-04-02 Violation(s) Noted Drilling appeared to have been completed at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Noted that the silt fence implemented as an erosion control below the site had been compromised. Specifically the fence had been undercut from the runoff flow below the well pad closest to where the access road enters the site. This resulted in an off-site transport of sediment.to an unnamed tributary of the North Branch of South Fork Pine Creek. Additionally the runoff/spring flow form the opposite side of the site had overtopped the silt fence and hay bales implemented as reinforcement resulting in an off-site transport of sediment to the same UNT. For this site a collection ditch and subsequent sediment trap would have been a more effective erosion control BMP. 584436
1877332 2010-04-14 No Violations Noted Drilling was completed at the time of the inspection. Noted that all of the issues noted and cited as violations in the 04/02/10 inspection had not been addressed.
1884108 2010-05-13 No Violations Noted Drilling appeared to have been completed at the time of the inspection. Met onsite with operator representative Tom Baun. Well was equipped for & in production. Brine tank located onsite and appeared to be in sound condition. Site had been regraded, however it had no as of yet been seeded and mulched. Observed that the erosion ditch that had been formed had been backfilled in, and the runoff flows had been eliminated. The silt fencing had been restablished and no accelerated erosion & sedimentation issues were observed in relation to the access road or well-site. Mr. Baun stated that the site would be seeded and mulched within the next few weeks. The operator should continue to monitor the site for accelerated erosion & sedimentation and to insure that the 70% uniform coverage criteria is achieved.
