| Inspection conducted on 2/4/14 of the Hinkley 2H estimated 20 gallon production fluid spill which was discovered on 1/31/14 and reported the same day.  Present during the inspection was Brody Webster and Jon Sassi.  The spill occurred when the dump line coming off the GPU froze and consequently caused a hammer union joint to loosen and release the reported amount of production fluid.  The 2H and 4H wells have been shut in.  Both GPU’s have been moved.  Resource Environmental is on site characterizing the extent of the spill and overseeing the remediation.

| Arrived at the site at 11:15 in response to a flowback fluid spill.  Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas was on-site during the inspection.  Mr. Watson stated that the material spilled to the pad surface is considered recycled water from Cabot’s A&M Hibbard centralized treatment facility on State Route 29 in Dimock, PA.  The recycled water is mainly a mixture of hydraulic fracturing flowback and production fluid transported to the site from Cabot well sites for treatment with intent for beneficial reuse during hydraulic fracturing operations at Cabot well sites. Vacuum trucks were being temporarily staged on the Hinkley Pad due to a lack of room on the nearby J Hawk Pad with hydraulic fracturing operations were in progress. The result of the spill, was due to a valve failing on the back of a parked vaccuum truck, according to Mr. Watson.  The amount of the spill is believed to be less than (10) gallons.

2243364 Primary Facility 02/04/2014 Routine/Complete Inspection No Violations Noted Inspection conducted on 2/4/14 of the Hinkley 2H estimated 20 gallon production fluid spill which was discovered on 1/31/14 and reported the same day. Present during the inspection was Brody Webster and Jon Sassi. The spill occurred when the dump line coming off the GPU froze and consequently caused a hammer union joint to loosen and release the reported amount of production fluid. The 2H and 4H wells have been shut in. Both GPU’s have been moved. Resource Environmental is on site characterizing the extent of the spill and overseeing the remediation.
2339740 Primary Facility 01/22/2015 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 11:15 in response to a flowback fluid spill. Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas was on-site during the inspection. Mr. Watson stated that the material spilled to the pad surface is considered recycled water from Cabot’s A&M Hibbard centralized treatment facility on State Route 29 in Dimock, PA. The recycled water is mainly a mixture of hydraulic fracturing flowback and production fluid transported to the site from Cabot well sites for treatment with intent for beneficial reuse during hydraulic fracturing operations at Cabot well sites. Vacuum trucks were being temporarily staged on the Hinkley Pad due to a lack of room on the nearby J Hawk Pad with hydraulic fracturing operations were in progress. The result of the spill, was due to a valve failing on the back of a parked vaccuum truck, according to Mr. Watson. The amount of the spill is believed to be less than (10) gallons.
2343379 Primary Facility 12/24/2014 Incident- Response to Accident or Event No Violations Noted The Department was on site at the R Hinkley well site located in Springville Township, Susquehanna County to conduct an Incident Inspection in response to a self-reported release of flowback fluid. At the time of the inspection, Cabot Oil & Gas was unable to have a representative present to discuss the specifics of the incident due to the holiday. The Department will return to the site when a Cabot representative is present to conduct a Follow-Up Inspection.
