| A fork truck punctured a tote containing 200 gallons of ferric sulfate.  All of the fluid was released to the containment.  The fluid was then removed by a vacuum truck.  The operator began to remove the felt over the liner for disposal.  At this time it was noticed that there was a breach in the containment.  At the time of the notification a 9’X12’ area was determined to be impacted. A 20’X30’ area was excavated and the surface of this area was scraped down between 2”-4” with a backhoe.  It was determined that the tote held (250) gallons and that an approximate 10-20 gallons were remaining below the puncture hole. The Department requested that a copy of the MSDS for KR-B6001 be submitted.

|  At time of inspection, arrived on site at 9:00 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 509psi on the production casing. I took a walk around the location and noticed repair work was done to the perimeter of the well site and everything looked OK.
 +
I left the site at 9:20 a.m.

| Inspection of the Hibbard A&M Pad 2 in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit. EPS, Jon Sassi, was also present on this inspection.
 +

 +
The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspectionPost Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans.  The well pad is less than or equal to the size indicated on the site restoration plans.

No violations were observed at the time of this inspection.
+
Based on the site inspection, I recommend this notice of termination be APPROVED.

|
+
Jon and I also conducted a general inspection of the pad. Signs and labeling appeared to be correct. The mailbox contained the required items. There were no apparent issues with containment. Conductivity readings of water on the pad were of no concern.

| Met with Cabot staff on the Hibbard A&M P2 to discuss vegetation prior to Cabot's submittal of the notice of termination of the Erosion and Sedimentation permit for the pad. Turnips were planted on the cut and fill slopes of the well pad as part of the restoration seed mix as a deer feed plot, not to be harvested. I didn't consider this an obstacle to receiving an approval of the notice of termination.

At the time of the notification a 9ï¾’X12ï¾’ area was determined to be impacted. A 20ï¾’X30ï¾’ area was excavated and the surface of this area was scraped down between 2ï¾”-4ï¾” with a backhoe. It was determined that the tote held (250) gallons and that an approximate 10-20 gallons were remaining below the puncture hole. The Department requested that a copy of the MSDS for KR-B6001 be submitted.

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1849767 Primary Facility 12/10/2009 Drilling/Alteration No Violations Noted At the time of inspection, drilling depth was 4922¿. Proposed TD is 9735¿. Company man on site indicated that surface casing was set at 1500¿ and floats did not hold during cementing. The Company man indicated that they shut in the well to hold the cement downhole.
1852136 Primary Facility 12/23/2009 Drilling/Alteration No Violations Noted Arrived on Site at 10:45 am. I met with Company Man Dave Moore. Mr. moore indicated that the had set 8721¿ of 5-1/2¿ production casing to TD. Float collar is at 8675¿. The production string was cemented with 1346 sks Type I Cement with .8% R3, 1% FL-62, .1% ASA-301, .005gps of FPS-6l, .8% BA10@14.8ppg , and had 1.34cuft/sak yield. Displaced 193 bbls of water during cementing. The plug was bumped on 12/22 and the floats reportedly held. The well is complete and is currently shut in. The rig is in the process of being dismantled and mobilized to the next location. Drill rig on site is Patterson Rig 154.
1880478 Primary Facility 03/22/2010 Complaint Inspection Violation(s) Noted CSL 401 black fluid discharge to surface and waters in area of Hibbard 2H & 4H well pad, 78.56(a)(1) tears in liner.

2047333 Primary Facility 02/22/2012 Incident- Response to Accident or Event No Violations Noted
2047344 Primary Facility 02/27/2012 Follow-up Inspection No Violations Noted
2102471 Primary Facility 09/24/2012 Routine/Complete Inspection No Violations Noted Specifics and details associated with this inspection report can be referred to in the A&M Hibbard 2H inspection report from 9-24-12. Permit number is 115-20149.
2156424 Primary Facility 04/10/2013 Administrative/File Review No Violations Noted A fork truck punctured a tote containing 200 gallons of ferric sulfate.
2393369 Primary Facility 07/28/2015 Routine/Complete Inspection No Violations Noted
2491183 Primary Facility 2016-06-08 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:00 a.m., the cellar was about three quarters full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 509psi on the production casing. I took a walk around the location and noticed repair work was done to the perimeter of the well site and everything looked OK.

I left the site at 9:20 a.m.

2530990 Primary Facility 2016-10-20 Site Restoration No Violations Noted Inspection of the Hibbard A&M Pad 2 in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit. EPS, Jon Sassi, was also present on this inspection.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.

Jon and I also conducted a general inspection of the pad. Signs and labeling appeared to be correct. The mailbox contained the required items. There were no apparent issues with containment. Conductivity readings of water on the pad were of no concern.

2531075 Primary Facility 2016-09-20 Routine/Complete Inspection No Violations Noted Met with Cabot staff on the Hibbard A&M P2 to discuss vegetation prior to Cabot's submittal of the notice of termination of the Erosion and Sedimentation permit for the pad. Turnips were planted on the cut and fill slopes of the well pad as part of the restoration seed mix as a deer feed plot, not to be harvested. I didn't consider this an obstacle to receiving an approval of the notice of termination.
2548678 Primary Facility 2016-12-05 Routine/Complete Inspection No Violations Noted On December 5, 2016 the Department, represented by Frank Hoehle was on site to conduct a Routine Complete Inspection of the Hibbard well pad.

The wells associated with this pad are as follows: 115-20149 115-20232

This pad was stable, with no erosion or sedimentation issues at this time. There was excellent vegetation all around the site, and a very nice crop of brassicas on the site as well.

There were no violations
