| On March 9, 2012, the Department received a complaint about alleging run-off from Chevronï¾’s Hartley site getting into a privately owned farm pond.  On March 14, 2012, WQS Bill Keefer, WQS John Ley and I investigated.
 
  
When we arrived at the farm, we found no one home but we walked down to the pond anyway.  The pond was still off-color and it appeared that there was some sediment in the small stream that fed it.
 
 
We were called off the site due to an emergency but returned later in the afternoon and stopped at Chevronï¾’s Hartley site.  Chevron was simultaneously filling a frack pond, vertical drilling a well and constructing the rest of the site.
 
 
There was an E&S plan in the tube in front of the site and a site sign.  We found several areas where the triple silt sock was over-run.  We also found bare areas where there were no E&S controls and areas sediment got off-site.  There were also bare slopes that did not appear stabilized within the prescribed 4 days.  These are all violations.
 
 
We left and I contacted Beth Smock about the problems.

| On 2/27/2014, the Department conducted an inspection at Chevron Appalachia's Hartley Well Unit 6, 7, 8, 9, 10, 11H site in German Twp., Fayette County.
 
 
The site ID sign was in place. The PPC plan and the E&S plan and the permit for the wells were in the mailbox.
 
 
The site's access road was in good shape.
 
 
The E&S controls were in place and looked to be functioning.
 
 
The pit on top of the hill by the guard shack and the pit on the pad have both been reclaimed and were mulched over.
 
 
There are 4 tanks on site located in containment.  There are 6 heater/separator units on site and the pipe for the heaters is located above ground.
 
 
There were 5 roll-off dumpsters on site as well as some equipment for the remainder of the reclaimation of the site.
 
 
Recent disturbed areas of the site had been mulched.
