| I inspected this well to check the idle status of this Berea Sandstone well.  The last commercial production reported from this well was in 1988 by D. A. Waldron & Associates and no commercial production has been reported since ownership was transferred to David A. Waldron in 2007.  My inspection found open 7" drive pipe and 4.5" production casing equipped with a 4.5" T-fitting.  A 4"X 2" swage and ball valve was on top and equipped with a gauge that was not readable.  A 2" flow line was connected to a bull plug on the T and ran to the east and underground.  The ID posted at the wellhead indicates D.A. Waldron & Associates as the owner and is incorrect.  No associated production equipment was observed on location.  Based on the fact that no commercial production has been reported from this well in 26 years, David A. Waldron will, within 180 days, obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code.  Failure to comply with this notice may result in further administrative action including issuance of a Chief's Order and bond forefeiture.
