| At 10:36 am on 3/10, I received a call from Dale Nething with OVE reporting an unknown failure at the well site and requesting my immediate response. I arrived on site at approximately 11:15 and met with Rick Liddle and Tom Wood. Universal Well Service was on site to frac the well and J-Well was on site with the service rig. According to Rick Liddle, the well broke at 1200 psi. Universal balled off and then shut down for 30 minutes for the balls to fall off. Universal started in with the nitrogen foam pad and had mixed and displaced approximately 2/3 of the planned volume (250,000 scf). The pressure was 2230 psi when the well started circulating gas out the backside. Universal shut down immediately and J-Well plumbed both the 4-1/2 and the backside directly into a 300 bbl frac tank. According to Tom Wood, the 4-1/2" casing came from McJunkin and was classified as limited service, 11.6 ppf tested to 4900 psi. The limited service designation was due to mixed R-2 and R-3 lengths. Reportedly, at no time was the backside shut in. The frac crew was released and Universal sent a pickup truck with a wireline spool and a pump truck to pump a flag down hole to try an locate the problem. Universal pumped 31 bbl and displaced the flag to 426'. The well never circulated fluid but was gasing (presumably nitrogen). The maximum pressure was 150 psi at a maximum rate of 4 bpm. The well was plumbed into a frac tank and left open over night.
