| 9:00 am Called Bakerwell's office and talked to Rex Baker Re: NOV's and Preliminary Restorations that are past due. Rex said that he never plants grass in pasture fields and did not think that the State had rules to that effect. I informed Rex of his obligations to establish vegetation on all well restoration sites. Rex then informed me that he lost his contractor in the area and is having a hard time finding someone to do the work (plant grass). I discussed my concerns about not mulching the site. I explained how the mulch not only helps to get the grass to grow mulch also helps hold loose soil in place to prevent erosion. Rex said that he will see what he can do in the next few weeks. I called Joe Hoerst and explained my conversation with Rex and Joe agreed to give him two weeks to get some grass growing before we go to the next level of enforcement.

| Well head is leaking some gas. There are bubbles around the well head. I have taken pictures. Rex Baker is going to try to tighten the head . He seems to think that will fix the leak. The pits have been filled in. No other restoration has been done at this time.

| Monitoring equipment site is located right off the highway. Flow gauges for the operation of this well are at this spot. The well's ID which is in the adjacent field is on the brine tank at the monitoring site. Also the telemetry hardware at this spot that relays operation of this gas well to Bakerwell office did not work unfortunately.  Met Perry Brock who is monitoring the field operations of Bakerwell producing wells was at this location when Jon and I stopped to inspect the well operation.

| No id. Site has been graded. Seeding is spotty concentrations in spots and no seeds on most of the site.  The site is bare ground. No mulch has been applied to the site. I talked to Bakerwell's contractor Jay Myers and I asked him about the mulch. Jay told me that Rex Baker told him not to mulch the site. There are still a lot of pit liner pieces sticking out of the dirt. All valves on the well are open and the gauge reads 0 psi. Without good vegetative cover I cannot pass the NOV.

1662072998 2007-09-12 Called Rex Baker today and left a message with his receptionist and on his cell phone voice mail for rex to call me Re: Restoration work on this well site.
1690107036 2007-09-14 I called Rex Baker and left a message on his voice mail that the grading looks good. But I cannot pass the restoration until grass is growing on the site. Also told him that the pit liner pieces need to be removed.
-1905652804 2007-10-05 The site has mulch applied to it. Good coverage. Some grass is starting to grow. No id.

-381860244 2012-10-04 Perry Brock on location working on remote telemetry equipment. Well location is in good condition . Good vegetative cover. Well is connected to production equipment and production line. Identification is on plastic tank. Accompanied by Morgan Button MRI 2.
-813587858 2006-12-26 Well head is leaking some gas. There are bubbles around the well head. I have taken pictures. Rex Baker is going to try to tighten the head . He seems to think that will fix the leak. The pits have been filled in. No other restoration has been done at this time.
-865282278 2012-10-04 Monitoring equipment site is located right off the highway.
-935679038 2007-05-23 No change, no seed or mulch on the site.
