| At the time of inspection, drilling depth was 4922¿. Proposed TD is 9735¿.  Company man on site indicated that surface casing was set at 1500¿ and floats did not hold during cementing.  The Company man indicated that they shut in the well to hold the cement downhole.   
 
 
Plastic has been placed in the area underneath the drill rig and under all associated drilling equipment on site.  Earthen berms have been installed around the rig, and around the perimeter of the well pad.

|  Arrived on Site at 10:45 am.  I met with Company Man Dave Moore.  Mr. moore indicated that the had set 8721¿ of 5-1/2¿ production casing to TD.  Float collar is at 8675¿.  The production string was cemented with 1346 sks Type I Cement with .8% R3, 1% FL-62, .1% ASA-301, .005gps of FPS-6l, .8% BA10@14.8ppg , and had 1.34cuft/sak  yield.  Displaced 193 bbls of water during cementing.  The plug was bumped on 12/22 and the floats reportedly held.  The well is complete and is currently shut in. The rig is in the process of being dismantled and mobilized to the next location.  Drill rig on site is Patterson Rig 154.

| A fork truck punctured a tote containing 200 gallons of ferric sulfate.  All of the fluid was released to the containment.  The fluid was then removed by a vacuum truck.  The operator began to remove the felt over the liner for disposal.  At this time it was noticed that there was a breach in the containment.  At the time of the notification a 9ï¾’X12ï¾’ area was determined to be impacted. A 20ï¾’X30ï¾’ area was excavated and the surface of this area was scraped down between 2ï¾”-4ï¾” with a backhoe.  It was determined that the tote held (250) gallons and that an approximate 10-20 gallons were remaining below the puncture hole. The Department requested that a copy of the MSDS for KR-B6001 be submitted.

1849767 2009-12-10 No Violations Noted At the time of inspection, drilling depth was 4922﾿. Proposed TD is 9735﾿. Company man on site indicated that surface casing was set at 1500﾿ and floats did not hold during cementing. The Company man indicated that they shut in the well to hold the cement downhole.

Plastic has been placed in the area underneath the drill rig and under all associated drilling equipment on site. Earthen berms have been installed around the rig, and around the perimeter of the well pad.

No violations were observed at the time of this inspection.

1852136 2009-12-23 No Violations Noted Arrived on Site at 10:45 am. I met with Company Man Dave Moore. Mr. moore indicated that the had set 8721﾿ of 5-1/2﾿ production casing to TD. Float collar is at 8675﾿. The production string was cemented with 1346 sks Type I Cement with .8% R3, 1% FL-62, .1% ASA-301, .005gps of FPS-6l, .8% BA10@14.8ppg , and had 1.34cuft/sak yield. Displaced 193 bbls of water during cementing. The plug was bumped on 12/22 and the floats reportedly held. The well is complete and is currently shut in. The rig is in the process of being dismantled and mobilized to the next location. Drill rig on site is Patterson Rig 154.
