| At the time of inspection the drill rig was onsite and placed on containment.  The erosion and sedimentation plan was onsite, however the department notes several areas surrounding the well pad, on the fill side of the pad that need routine maintenance, specifically regraded seeded and mulched.  the Department observed multiple areas that accelerated erosion cut channels and is transporting sediment toward the LOD and perimeter controls.  The perimeter controls appear to sound however the areas contributing to the accelerated erosion are in need of attention.  The Department suggest the Operator address the erosion channels and regrade, seed and mulch the affected areas.  The various pit located onsite appear to be in sound condition and appear to have the required 2' of freeboard. The Department also suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site maintained.

| At the time of inspection the well was in the process of bring stimulated.  The E&S plan was onsite and appeared to be installed as  outlined in the plan.  No sign of accelerated erosion and sedimentation in relation to the well pad or access road at the time of inspection. The Department notes that the drill pit located onsite has been back filled and the stock pile from which the material came to backfill the pit has been hydro seeded and appears to be stable.  The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
