1793911 2009-04-15 Violation(s) Noted Well location located by GPS unit. Date of Permit is 7/7/94. According to records, well drilled 8/12/94 to a depth of 9,436 ft from grade. No WACR report submitted to DEP. During inspection, well tag was not present at the well head or production equipment. The well was completed with 13" flange well assembly retrofitted to 4 gate valves, fluid relief valve and reduced to 2 inch production lines. Pressure readings on the dual pen data logging unit showed 200 psi on outside pressure and inconsistent readings of 25 psi on inside pressure. Site also contained separator, heating unit and brine tank (near well head and abandoned tank near road). The valves were located in the on position. Possible signs of fluid leakage occurring around the abandoned tank wellhead.

Called Everest about the release of gas. Pressure readings on the dual pen data logging unit showed 270 psi/10 psi. Site also contained separator, heating unit and brine tank (near well head and abandoned tank near road) and new brine tank present to replace the abandoned tank. Four 55 gal drums remain near production equipment and two 55 gal drums near well head. No E&S or IW issues at the site.
